29 May 78 - Crashed on 29 May 1978 Memorial Day while taking off in a 2-ship formation from McChord AFB on a routine cross-country flight to Tyndall AFB, FL. Pilot Captain Bruce Miller survived. Crashed on take off into a pond. The pilot tried to fly to an open field east of the base but crashed near 112nd Apartments east of air base due to an incorrectly installed main engine bearing.

59-0144 was a Class A mishap (no landing gear hydraulics), occurring at Castle on 19 Jan 62. However, it did not result in the loss of the aircraft. The aircraft was repaired, and eventually was transferred to McChord where it was involved in 4 other Mishaps; 2 in 64, 1 in 67, and 1 in 70 before being destroyed on 29 May 78 at McChord AFB when the engine failed on take-off (main bearing installed incorrectly).
