24 Jun 59 - To 498th FIS Geiger Field WA 28 Nov 60 - To 498th FIS, 84th FG, Spokane IAP WA 04 Feb 63 - Crashed. Flamed out during approach...headed to open fields before ejecting. Crashed at the foot of Jump Off hill south of Kennewick. Flew away from populated area of Trie-Cities before attempting to eject. Captain William D Neville, Missoula - ejected. Bill Neville was one of the few that had a successful ejection with the F-106 "B" seat. The reliable Weber seat went into service later.

Fuselage cut in half at station 412 (Aft bulkhead missile bay) and a new fuselage, cockpit section, and nose section was installed with the latest production avionics, the same as the last F-106A 590148 and F-106B 590165. A total of 35 aircraft (28 "A" models and 7 "B" models) were converted and reassigned to various ADC units..
