12 Oct 66 - Crashed and burned after entering the landing pattern 16 miles north west of the base. Pilot and commander of 5 FIS Col. John H. Fowler, Jr. killed in crash, no ejection.

The crash site was Carpio, ND, about 25 miles northwest of Minot ND on US Highway 52, and almost directly west of Minot AFB. The Crash Investigation Team said that pilot fatigue played a big role in the accident, that Col. Fowler may have seen the lights of the base in the distance and knowing he was almost home leaned back and relaxed: They said the final few seconds of any trip, when you know you're almost home, is the most dangerous part of any trip.
