06 May 60 - To 5th FIS, 32nd FG, Minot AFB ND 17 Jan 61 - To 5th FIS, 32nd FW, Minot AFB ND 22 Jan 62 - To 5th FIS Minot AFB ND 03 Oct 63 - Crashed: FCF Pilot Capt Wayne D. Wendt killed in crash. He was coming back from an FCF on a bird that had just come back from depot, pitched out, tower told him that his nav lights werent on, and the bird kept rolling & impacted the ground. No ejection, pilot was killed. Broughton went out to repeat the exercise & nearly augured in himself. Accident board did not decide if it was spatial disorientation (30 minutes after sunset) or FOD / Flight control problems.

Fuselage cut in half at station 412 (Aft bulkhead missile bay) and a new fuselage, cockpit section, and nose section was installed with the latest production avionics, the same as the last F-106A 590148 and F-106B 590165. A total of 35 aircraft (28 "A" models and 7 "B" models) were converted and reassigned to various ADC units..
